ASSESSMENT ONLY: $55 per assessment Assessment included FREE with Standard Service

LIFE SKILLS: Life Skills lessons in TLC Learning Portal included FREE with Standard Service

VA K-12 LEARNING ACCELERATION GRANT: Standard Service includes 2 sessions per week + the option to add more sessions (each session is $31.25 per hour/session) IMPORTANT: You must be approved and awarded the grant before signing up with TLC Math & Reading. If you have not been awarded the grant, please go to VA K-12 Learning Acceleration Grants to apply. It takes about 15 minutes to complete the application. The grant is awarded on a first-come, first-served basis and takes about 2 weeks to receive. Please return here once you have been awarded the grant. If you have already received the grant for your student, please select "Yes" to the question above question and click Submit. You will receive next step instructions within 24 hours of submitting this form.

I authorize TLC Math & Reading to debit my account for the first month's (or week's) subscription fee. Thereafter, I authorize TLC Math & Reading to debit my account each month on or around the monthly or weekly anniversary date for the agreed upon amount. This draft will remain in effect until I cancel my subscription (30-day cancellation notice required for monthly subscription; 7-day cancellation notice required for weekly subscription). My student and I agree to adhere to the terms and conditions of TLC Math & Reading Parent & Student Policies Handbook.

VA K-12 LEARNING ACCELERATION GRANT: The services are at no cost to you. The State of Virginia will pay for weekly invoices through grant funds. No need to provide a card number unless you would like to continue services after grant funds have been used.
